How YOU will make a Difference:
The Case Manager performs monitoring of member utilization and claim patterns and provides ongoing case management support for members engaged in the Steerage Program. The Case Manager also collaborates closely with internal teams including non-clinical members of the Med Steerage team and cross-functional nursing teams. The Case Manager also works closely with external entities including members, member care teams and Stop Loss partners for the purposes of coordinating care related to steerage opportunities.
What YOU will do:
Utilization Review

  • The Steerage RN will perform Utilization Management for potentially steerable cases, and work to identify steerage opportunities within the scope of the Steerage Program and guidelines. Reviews are conducted using approved criteria for appropriateness of services, setting/level of care and length of stay.
  • Where appropriate, the Steerage RN will collaborate with the Steerage team members to transition care within the guidelines of the Steerage Program.


Case Management

  • The Steerage RN demonstrates skill in case management and core nursing processes including assessment, planning, implementation, coordination, monitoring and evaluation of proposed treatment plans.
  • Provides ongoing review of treatment progress as needed to provide personalized support and care coordination for complex, catastrophic or ongoing chronic medical conditions.
  • Evaluates services requested to meet an individual's health care needs, with the goal of providing continued personalized case management to promote and ensure continuity of care coordination.


Knowledge, Experience, and Key Attributes needed for Success:

  • Current Baccalaureate prepared (Preferred) in Nursing or Community/Public Health
  • 3-5+ years of clinical nursing experience
  • Active Certification or RN clinical license, as applicable to the degree
  • Background in Oncology/Infusion strongly preferred
  • 3+ years of utilization review experience (specialty medications)
  • Problem solving and critical thinking skills
  • Knowledge of Case Management and Utilization Review processes
  • Excellent, client-facing verbal and written communication skills
  • Ability to be self-motivated and self-directed
  • Enjoys the pace and rhythm of a deadline-oriented environment with strong prioritization skill sets
  • Enjoys working in a collaborative environment
  • Proficiency with Microsoft Office applications (Outlook, Word, DOSS)


Compensation:
The base salary range for this position in the greater Seattle area is $88,000 - $107,000 and varies dependent on geography, skills, experience, education, and other job or market-related factors. Performance-based incentive bonus(es) is available. HMA is primarily a remote workforce, requires a reliable internet connection, and provides all hardware. Some roles are required onsite 1-3 days per week/month while other roles are 100% remote.
